The Importance of Updating your Child's Medical History

 

 
In the healthcare industry, up-to-date medical history information is essential. A timely update isn't just a formality, it's a key part of ensuring a smooth healthcare experience for both patients and providers. Here are a few reasons why.

Preventing Authorization and/or Referral Denials

The pediatric medical history information that you provide through our portal will be listed on your child's evaluation and re-evaluation reports that the clinician completes.

Your insurance company requires a detailed evaluation report, including medical history, prior to approving any authorizations or referrals. If the medical history that you provide is not up to date, this gives insurance reason to delay authorization approval, or even deny authorization requests.

Help us to avoid delays in authorization and/or referral approvals, by ensuring your medical history is updated at a minimum, bi-annually.

Changes in Patient Information

Some items that are included in your child's medical history change often. For example, medical history requires that you list your child's current grade level, which school your child attends, any services your child receives outside of CEPT, your child's current diagnoses, and all the medical providers that your child sees regularly, etc.

By reviewing your child's medical history on a bi-annual basis, you can easily update this information as it changes.

Ensuring Accurate & Efficient Treatment Planning

Think of updating your child's medical history as the key to a more accurate and efficient treatment experience. By ensuring that medical history has been updated to the best of your knowledge, your child's clinical team has a more holistic and accurate overview of your child's areas of growth and development. This is a quick way to ensure that you have updated your child's clinical team with any changes that may have occurred.

Some parents express worry that the time they spend updated medical history will be a waste, and that no one will review the changes they make. Our clinical team reviews your child's medical history every 6 months when they complete a progress report and/or re-evaluation to check your child's progress with established goals. Please take a moment to update your details so that we can provide your child with the best care possible!

What You Need to Do
 
Login to your Patient Portal

Go to our website: www.cuttingedgepediatrictherapy.com & click on the patient portal icon, it will open the portal page & outline items that you can access online.

Login to the patient portal using the email address that you have on file with CEPT. Forgot your password? No problem! Select "Forgot Password?" and follow the prompts to change the portal password.

The Patient Portal will allow you to:
  • View and print details of your upcoming appointments.
  • Fill out patient demographics, medical history, & consent forms.
  • Access to evaluations and daily notes for ST, OT, PT, & counseling sessions.
  • Confidently submit new patient information securely from the comfort of your own home.
Update Medical History

Once logged in to the patient portal, select Records, then Medical History.

From there, you can either edit the existing pediatric medical history or add a new pediatric medical history.

*Tip: by editing a pre-existing pediatric medical history entry, you can easily update anything that has changed, and you won't have to fill in all the details that have not changed.

You will notice that the medical history has multiple tabs. We ask that you provide and update as much information as possible, as it will help our clinicians prepare and understand the patient's general history, along with your updated concerns. The information provided will be your contribution to the patient's comprehensive clinical evaluations, re-evaluations, and treatment plan.

Update Patient Demographics

After updating the pediatric medical history, we recommend verifying that the most up-to-date information is listed in the patient's demographics and contacts.

Select Profile, then Personal Info to verify and/or update patient demographics. 

Then, select Contacts Info to verify and/or update the patient's referring doctor, primary care physician, caregiver contacts, and/or emergency contact.
